Subjective, Objective, Assessment, and Plan

SOAP refers to the way progress notes should be organized. Subjective information is an account of a health problem given by a patient. Objective information includes lab results and a physical examination. Assessment is the summary of all clinical information or physician conclusions. A progress note should end with a plan for patient care, including any diagnostic testing.
