Dehiscence is the simple separation of the surgical incision, while evisceration involves the protrusion of the internal organs through the dehisced incision. Both can be life-threatening medical emergencies. The nurse should remain with the patient, notify other staff, and hold light pressure of the area with a sterile, damp towel until the surgeon arrives.
