Providing continuity of care is one the essential components of nursing care. In addition to all of the above, continuous and, sometimes, long-term evaluation and reassessment of the patient’s treatment goals and needs are part of this process. Continuity of care helps to ensure that the delivery of care is seamless, appropriate, and efficient and that it meets the patient’s immediate needs as well as long-term wellness goals.
