All four criteria must be met in order for informed consent to be obtained. The nurse serves as a witness and facilitates informed consent by ensuring that the patient is competent, has the proper information (answering questions, providing translators, etc.), and that the entire process (including patient refusal of treatment) is properly documented in the medical record.
