Nurses play a key role in continuity of care as they are the primary communicators for their patients. They must ensure that all the necessary documentation is included in the patient’s chart and follow up on and communicate any unresolved issues (labs or test results) to the appropriate person overseeing the patient’s care. This requires awareness of the specific role of each person on the care team.
